New Patient Forms
Why waste time in the waiting room filling out forms? Download, print, and fill out all patient information paperwork at your convenience by clicking the links below. Save the PDF files to your computer, then print them out at home.
When you visit us, bring the completed forms with you. We will promptly register you, so you can get on to your appointment with Dr. Rassiwala as quickly as possible.
Payment Options
We don’t want finances to stand between you and the healthy smile you deserve. We accept a number of different payment options, including personal checks, money orders, and most major credit cards. Financing is also available through CareCredit™.
Payment is typically required at the time the procedure is performed. However, if your treatment requires more extensive dental work, contact our office to discuss your payment options.
Dental Insurance
